Welcome to the Sheffield Teaching Hospitals NHS Foundation Trust Medicines Formulary
This Medicines Formulary has been developed to reflect:
- good evidence-based practice
- good economic practice
- national guidance
- current therapeutic opinion
The STH Medicines Management and Therapeutics Committee (MMTC) is required by the Trust board to produce, uphold and review prescribing policies for the Trust. This Formulary has been produced under the auspices of MMTC as part of that remit. Medicines that are routinely available from pharmacy for general or restricted prescribing are included. The Formulary should be used in conjunction with Trust / Clinical Directorate guidelines where appropriate.
New medicines may be considered for inclusion in the Formulary through application to MMTC. The relevant documentation for this process is located here along with Committee minutes and terms of reference.
This site provides the following:
- fast access to information by drug name, class, condition and recent amendments
- first and second line recommendations as appropriate, along with relevant supporting information and guidance
- details of restrictions
